What happen to this blade causing it to fail?

One last question I have about this topic. Do you think the blade failed because it had a flaw from the factory or do you think it just develop cracks over a long period of time? Do you think the intact blade on the other side may have these cracks developing as well? Just wondering if I installed a new blade on one side and continue to use the cutters with the old blade do you think the old blade would fail sometime in the near future?
Given these two blades did the exact same amount of cuts but one side failed means the failed blade was weaker from the start?
 
I say its just over time failure.

Changing its jaws, Hard to say if its companion is cracked. Only inspection will show that.
I might as well change both jaws. Last time I bought HKP jaws, they were a pair.
Be sure you set the adjustments right...:p
