Best USA made neck knife under 4.5 inches?

Assuming you are talking 4" blade length, I like my BK-11 san mai.

Runner up would be the TOPS/Buck CSAR-T Liason.

If you want high end, a BUSSE Bear Cub or a Hinderer Flashpoint.
 
Bark River Bravo Micro is a good one. So is their necker. Do you want a traditional styled knife? Meaning a regular handle blade config? If not and you are open to other styles, the Survive Knives Necker is great. I have two. Also the Spartan Blades Enyo is nice.

Not sure if you mean OAL, but I have a Daniel Fairly (member here) Backpacker in Ti. It's right at 6" total, 1/16" thick. I keep mine on my backpack but I'd imagine it would be almost imperceptible hanging around your neck.

I'm a huge fan of the Tops Tibo. I bought one back in April to replace my Izula and I fell in love. Even without a cord wrap, it stays comfortable and secure, comes with a nice Kydex sheath, and weighs in at 1.9 ounces. It's my primary EDC when I'm around town, and it's my backup when I'm in the woods, where my primary knife becomes my ESEE 3.
 
Check out Fall Creek Knives. Shannon has recently come out with a new smaller knife called the Urge and they are awesome. Shannon and Bob Carter are both members here and great to deal with.
 
Dpx gear hest and hest II, both work well in that scenario with the scales removed. Fallkniven kolt, wm 1, or the juni are great knives with excellent steels. There really is no end to the options available here so find one that's visually appealing to you, check out its.specs to see if meets your wants n go from there
 
The most useful neck knife I own is the Outdoor Edge Wedge. It is 5 inches long but still manages a 2 1/2 inch blade. The next best is the Cold Steel Super Edge which is about 4.5 inches in length. I have a box of neck knives (I went through that phase) and these two are my favorite. My Izula gets pocket carried (try running with one around your neck and see why).
