Modeling custom scales

Hey guys!

I got a question that' been bothering me longer than it should.

How do makers, like flytanium, get the exact dimensions of knife scales?

Let's say I have access to a CNC mill, how would I go about getting the CAD model or just the mechanical drawing (is it a legitimate word?) to generate the g-code etc.? Is there a special process to meassure the scales or do manufacturers hand out the drawings with dimensions?

I hope that my question makes sense.
 
Saw Russian guy on the tube who makes scales use a regular scanner to get his 2D down. He imports it into his 3d program and goes from there. Can't remember well, but I think he used some chalk on say black scales in certain places so that some needed lines are revealed show up on the scans.
