I had a very minor problem with the new left handed sheath for my 119, I came up with a very easy fix and because it was so minor with such an easy solution I'm not complaining at all.
My problem with it is that the plastic is just folded in half, this crease which the spine doesn't fit into pushes the edge against the welt cutting into it and makes the fit a bit tight.
It turns out it just slides out however removing it leaves too much wiggle room for the blade so my solution was to cut the end off and inserted it backwards.
The edge actually fits into this crease in the plastic unlike the spine, this covers and protects the welt, and since the spine has more room it loosens up the fit perfectly.
I just figured I'd throw this out there, because I had assumed the liner was probably glued in or something.
I don't imagine this would be a common issue if it wasn't in reality just me , but luckily if it does occur as a problem it's an extremely fix.
I also needed a solution for the belt loop that just didn't carry how I prefer.
I added some ranger bands just above the Buck logo to tighten the loop up some, and run my belt behind what is supposed to be the loop.

Now it's perfect, and only took mere minutes.

Im not complaining, just happy that I could tweak this to suite me in no time at all.

Glad you found your solution.

My dad used to make sheaths from PVC pipe. Heat with a heat gun, squish, insert knife heat squish again, trim. Repeat as nescesary. You could stick the whole sheath into you belt, or add loops, or even a clip.

One time I needed a liner, for a nylon sheath I found some plastic candy canes that were lawn decorations. I followed his lead. Cut to length heat squish around knife blade then glue into sheath. Worked good.
 
For the belt loop, I've been considering converting my 119's sheath to a dangler. I think a quick link for a chain (from Home Depot, Lowes, or a hardware store) would work, then make a new double loop from an old belt. The loop that the belt you're wearing goes through can be made tight enough that it won't slide.
I was also thinking of maybe a Baldrick or shoulder strap long enough to go over the off-shoulder and put the knife low enough to be at or below the belt line (so it is not a concealed weapon when wearing a coat) to wear with say, a snowmobile suit, or coveralls/overalls that do not have belt loops.
I might make both the belt loop and shoulder strap, so I have both warm and cold seasons covered.

I personally can't stand danglers, I like a tight sturdy belt loop that keeps a knife static and exactly where I put it.
 
I believe the insert is provided to prevent the slightly upswept clip point on many of bucks knives from poking through the leather at the front of the sheath. This can happen if the knife isn't inserted perfectly straight. If there was a welt at the front of the sheath instead of the folded over leather this wouldn't be a problem. As far as I know buck only includes the plastic insert in the leather sheaths with clip point knives. My 113 and 192 don't have them but my 119 and 105 do. It's not uncommon to see older 119s or 120s on eBay or YouTube with slices right where the clip comes in contact with the folded over leather at the front of the sheath. The welt, by design, shouldn't really need to be protected from the blade edge in a properly assembled sheath.

I was probably just over thinking this or something, because that seems like the perfect reason to have this in there.
