I just was given this old cleaver by my nighbors widow. I love the weight and could use it around my kitchen for venisen butchering. However the handle is in BAD shape. Any ideas on where I could send this to be repaired??

cut off the old wood and replace it.

Fairly easily done.

the rivets look to be made of brass and should be easily removed as well.

You can get handle slabs from most knifemaking companies (I like Alpha Knife Supply among others). Decide whether you want synthetic (micarta, G10) or natural (wood, bone) material and go from there.

I would personally go with pinned construction on your re-handle. It will eliminate the need for countersinks. Pin stock is also easy to get from knife supplies. The pins minimize shear stress while high-quality adhesives actually hold the scales on.
