+1 on the Maxped Sabercat and, if you want something smaller, the Proteus.

With both, I use Maxpedition's small rollypoly to hold my water bottle.

Same here. The Mini Rolly Polly fits a 32 oz. Nalgene perfect. Adds a lot of weight though. (Full Nalgene that is)
 
Aw man... I own a lot of Max gear but for some reason have never noticed the Proteus. May have to add one to the collection. Cheers!
 
My Versipack is the Proteus (IIRC). No water bottle holders but it has everything else the OP wants. I keep mine in the car ready to go. Makes a nice pack for day hikes and gives several attachment options for my Izzy 2 with a molle clip.
 
The Mountainsmith bags are great. You can fit some molle gear on the two webs where the bungie cord goes through as long as they are not huge heavy things.
