Just wanted to get input from leather pros. What is the best way to keep old leather in good shape ?

Assuming the leather is clean just a really light coat of pure neatsfoot oil every six months to a year should do it. The timing depends on the level of use the item gets. The less the use the longer the period between the applications of the oil. Always lightly dust the article to make sure you are applying to a clean surface. If the article has become dirty through use or otherwise then a thorough cleaning using saddle soap should be done and allowed to dry thoroughly before the oiling.

Paul...Thanks for the response. The sheath in question is from the mid 60's and has a fold down area that has a couple cracks in it. I am just trying to keep it from getting any worse.
